Everything in one place, split by who you are. Healthcare professionals will find the onboarding packet for becoming an authorized OptitrimMD provider. Patients on the program will find the guides, recipes and provider information that support each phase.
OptitrimMD is a high-protein meal replacement program that helps patients reach their weight-loss goals while maintaining lean muscle mass. Licensed healthcare professionals who want to dispense OptitrimMD in their practice complete a short onboarding packet before an account is opened.
The packet has three parts:
Complete the card form, the ACH form, or both, depending on how your practice prefers to pay. Still deciding whether the program fits your practice? Start with Partner with Optitrim MD.
These two forms carry card and bank account details, so they are handled on a private channel rather than filled in on the website. That is a deliberate choice about how your practice’s financial information is collected — it keeps those details out of ordinary website traffic entirely.
Returning the completed forms: fax the signed forms to 847.806.0464, or call +1 (630) 768-6784 to arrange a secure alternative. Please do not send card numbers or bank account details by email.
What happens next: complete the forms, sign and date every agreement, return the packet, and the OptitrimMD Provider Relations Team will confirm when your account is approved. Once approved you can begin offering OptitrimMD products to your patients.
These are the resources your provider will most often point you to.
Questions about your dose, your medication, your lab work or any symptom belong with the clinician supervising your program, not with a general guide.
Licensed healthcare professionals who want to offer clinically formulated meal replacement products as part of a physician-supervised weight management program. Start with Partner with Optitrim MD, then complete the provider application.
By design. Those two forms carry card and bank account details. A public web form stores every submission in the website database and emails a copy, which is the wrong place for that information. Download the PDFs, sign and date them, and return them by fax to 847.806.0464 or by another secure method we arrange with you.
No. Complete the credit card authorization, the ACH authorization, or both, depending on how your practice prefers to pay for product orders.
The OptitrimMD Provider Relations Team reviews the completed packet and confirms when your account is approved. Once approved, you can order and dispense OptitrimMD products in your practice.
